چکیده

The use of an elevated-rim acetabular liner in total hip arthroplasty (THA) is widely accepted. We sought to determine quantitatively the amount of additional stability provided by the elevated-rim liner as compared to the standard non-elevated liner. Furthermore, the stability of the hip with a 32-mm femoral head was compared to the standard 28-mm head. Our results show that a 15° elevated-rim acetabular liner placed in the posterior quadrant increased hip stability by an additional 8.9° of IR. Similarly, the 32-mm head provided 8.1° of additional IR. The increases were statistically significant (P < 0.0001). In addition, we did not encounter increased anterior dislocation intra-operatively. The findings of this study indicate that the 32-mm head may contribute to hip stability in primary THA, and in instances where a posterior approach is used, an elevated-rim liner placed in the posterior quadrant may also independently contribute to hip stability.

Effect of elevated-rim acetabular liner and 32-mm femoral head on stability in total hip arthroplasty.

OBJECTIVE Although the theoretical attractions of the elevated rim are obvious and have been widely accepted as a mean to improve the postoperative stability, the clinical advantages have not been demonstrated. The aim of this study is to further evaluate the elevated liners contribution to stability. METHODS Forty-six patients with 50 hips undergoing primary total hip arthroplasty (THA) were...

Highly Cross-linked Polyethylene Liner Dissociation from a Cement-less Modular Acetabular Shell: Two Case Reports

Liner dissociation of polyethylene from a cementless acetabular socket following total hip arthroplasty (THA) is a rare complication. Cross-linked polyethylene liner dissociation from AMS-HA shell (KYOCERA Med, Osaka, Japan) occurred in 2 out of the 4153 (0.04%) cases approximately 10 years after undergoing surgery at our institute. First case was an 80-year-old female who underwent right THA a...

A New Automated Way to Measure Polyethylene Wear in THA Using a High Resolution CT Scanner: Method and Analysis

As the most advantageous total hip arthroplasty (THA) operation is the first, timely replacement of only the liner is socially and economically important because the utilization of THA is increasing as younger and more active patients are receiving implants and they are living longer. Automatic algorithms were developed to infer liner wear by estimating the separation between the acetabular cup...

A modular femoral neck and head system works well in cementless total hip replacement for patients with developmental dysplasia of the hip.

We compared a modular neck system with a non-modular system in a cementless anatomical total hip replacement (THR). Each group consisted of 74 hips with developmental hip dysplasia. Both groups had the same cementless acetabular component and the same articulation, which consisted of a conventional polyethylene liner and a 28 mm alumina head. The mean follow-up was 14.5 years (13 to 15), at whi...

Damage of an Oxinium femoral head and polyethylene liner following 'routine' total hip replacement.

We present a case of early retrieval of an Oxinium femoral head and corresponding polyethylene liner where there was significant surface damage to the head and polyethylene. The implants were retrieved at the time of revision surgery to correct leg-length discrepancy just 48 hours after the primary hip replacement. Appropriate analysis of the retrieved femoral head demonstrated loss of the Oxin...
